End of Module Survey

Thank you for completing the COVID-19 vaccine curriculum created to support health professions students in the clinical practice environment. Given all that you have learned, it is our hope that you will use the information and tools to positively impact vaccination in your respective practice environments and communities serving as vaccine ambassadors and in some cases as vaccinators.

Our goal is to continue to develop and enhance this curriculum therefore we want to hear your feedback. Please take a few minutes to respond to the questions in the following survey about your experience. 

The survey contains 15 questions and should take about 5-10 minutes to complete.  Please note that this survey is completely voluntary and your responses will be kept anonymous. The information you provide will be shared with the COVID-19 vaccine curriculum working group for the purpose of improving the curriculum. Thank you!
